Other Methodist Policies Regarding Insurance
- Methodist does not accept automobile or liability insurance. However, can you please reach out directly to Methodist to see what their Insurance Policies are. You can reach out to the following Methodist personal to get the accurate information: akrobertson@houstonmethodist.org, lywalters@houstonmethodist.org
- Worker's Compensation. If you were injured on the job please inform our scheduling staff at the time you make your initial appointment.
- At the time of your visit, you will be asked to sign a Disclosure of Information form. This will allow us to obtain any previous medical records not available at the time of your visit.
You will need the Adobe Reader to view and print these documents.
